Physical therapist must be able to identify signs and symptoms of systemic origin that can mimic neuromuscular dysfunction.
Sacrum and sacroiliac joint.
- No single physical examination finding can predict a disorder of the SI joint.
- Pain originating from the SI joint can mimic pain referred from Lumbar disc herniation, spinal steno-sis, facet joint dysfunction,or even a disorder of hip.
- A fall on the hip, lifting of heavy object in a twisted position, childbirth.
Typical systemic diseases that refer pain to the sacrum and sacroliac joint; gynecologic, prostate cancer.
Risk factors And clinical presentation....!
Osteoprosis, sexually transmitted infection, long term use of antibiotics (colitis).
Lack of objective findings, Anterior pelvic, suprapubic, or low abdominal pain at some level as the sacrum.
Spondylogenic causes of sacroiliac pain.
Metabolic bones disease (MBD)
>Osteoporosis,
>paget's disease,
>osteodystrophy
Can result in loss bone mineral density and deformity or fracture of sacrum.
A bone disease occurs when the kidneys fail to maintain the proper levels of calculam and phosphorus.
back pain may be referd from systemeic or nueromuscoulostetal originiation from hip, SI joint, sacrum, pelvis, low back, groin, thigh.
Aggravating and relieving factors :
Aggravated by exercise, weight bearing, Lumbar Rotation. Relived by rest or stretching.
Stress fractures during traumatic delivery, Osteomalacia and Paget's Disease.
